Palliative Care is a specialized holisitic medical care provided by an interdisplinary team. This team provides an additional layer of support for persons of all ages with a chronic illness. Palliative Care is appropriate for any person who wishes to continue with aggressive treatment of their illness. Palliative Care can start at time of diagnosis and continue through the disease process. This service is covered by most insurance plans, including Medicare and Medicaid.

The team assists:

  • Advance care planning (i.e living will and establishing medical powers of attorney). Click here to learn more about Advance Directives. 
  • Goals of care and symptom management for those with chronic illness
  • End of life discussions
  • Psychosocial-spiritual counseling 
  • Support for patients and their families

We can also provide assistance with transitions to hospice if a patient has decided to stop seeking aggressive treatment.

The Palliative Care Team is comprised of a Medical Director, Family Nurse Practitioner, Registered Nurse, Chaplain and Social Worker. St. Peter’s Health Palliative Care team provides inpatient and outpatient services. You can view a pallitative care brochure by clicking here.
