In support of 2017 MT Code Annotated Chapter 1, Title 41-1-401-41-1-403, St. Peter’s Health does not provide electronic access to health care information for minors (patients age 13-17). Electronic access via the Patient Portal ends automatically on a patient’s 13th birthday. You can continue to obtain your child’s health care information through their primary care provider or by visiting a Medical Records Department at any St. Peter’s Health location.

To obtain printed copies of your child's health care information, you must fill out a Release of Information (HIPPA) form and visit a Medical Records Department at any St. Peter’s Health location.  You will be asked for proof of your identity.

Access for permanent legal guardians and medical power of attorneys will only be provided if appropriate legal documents are attainable in the child’s health care information or you provide these documents at time of registration.

To obtain electronic access to your child’s health care information when you are a St. Peter’s Health patient, you must also have your own Patient Portal. If you do not have a Patient Portal, you will fill out a Patient Portal Registration form in additional to filling out a Patient Portal Proxy Registration form. If you have a Patient Portal, you will only fill out a Patient Portal Proxy Registration form.

To obtain electronic access to your child’s health care information when you are not a St. Peter’s Health patient, you must have a Non-Patient Portal. The only form you will fill out is the Patient Portal Proxy Registration form.

Forms must be signed in the presence of a St. Peter’s Health employee after they have validated your identity.
