Due to new federal requirements, St. Peter's Health is no longer required to automatically furnish past or present employees with a 1095-C. However, a copy of your 1095 is available to view and download electronically from the Paperless Employee portal.
Once you access the portal, please authenticate your identity by logging in with a username and password and then download your Form 1095-C. If it’s your first time, you will need to create an account. This step-by-step guide is available to help you through the process.
