New and Transferring Patients

Step 1: Ask your referring provider to fax your information

To begin establishing care at the St. Peter’s Health Infusion Center, please contact your referring provider and ask them to fax the following information to 406-495-6853:

• Infusion orders
• Insurance authorization paperwork
• A copy of your current insurance information
• Notes from your most recent clinic visit 

Please allow three business days for our team to receive and process your information before contacting us for a referral update. Having these documents on file helps our team review your referral and better assist you. Once we have received and reviewed your referral information, we will contact you with next steps. 

Please note: Chemotherapy orders are not accepted at the Infusion Center. If you are looking to transfer your oncology care, please contact St. Peter’s Health Cancer Care at 406-444-2381.

Step 2: If needed, contact our team with questions or for a follow-up

Once your referring provider has faxed your information and you have allowed time for processing, you may contact any of the team members below to ask questions or follow up on your referral.
